Each Public Utility, as defined in section 16-345 of the Connecticut General Statutes, is required to report excavations that occur in violation of sections 16-345-1 to 16-345-9, inclusive, as well as any damage to its underground facilities that result from excavation or demolition. Excavators may also, voluntarily submit these reports. Connecticut Statutes allow PURA to levy a civil penalty of up to $40,000 for failure to comply with any provision of the statute or regulations. These reports may be used in actions imposing a civil penalty for such violations. Therefore, complete these forms ACCURATELY and submit them no later than the 15th of the month following the violation or damage.
